How do I update my patient's Care Team?

Patients  may be assigned to Group(s) and Team Member(s) according to their affiliations within your organization. It is required that patients be assigned a group. It is optional that patients be assigned a Team Member. Therefore, it is possible that patients exist in a group without yet being assigned a Team Member (coach or other provider) in both the Enrolled and Not Enrolled state. Twine Patient Status Definitions are available here for context. This article outlines the steps to update a patient's Care Team.

 To open the Care Team Settings

  1. Go to Profile > Care Team, or, click on the Care Team Icon within the Patient Vitals area.

    A slide out will appear on the right hand side of the screen for you to update Group or Team Member affiliations.

To add or change a patient's Group:

  1. Click +Select Group and choose an available group from the drop down. Screen_Shot_2017-10-31_at_4.26.49_PM.png
  2. It is required to have a group associated with your patient's Care Team. Once you have two or more groups affiliated with the patient, you may reveal the option to Remove Care Team so that the groups reflect the current affiliation of the patient. Screen_Shot_2017-10-31_at_4.31.00_PM.png
  3. Once you are satisfied with your choices, click Update Care Team.

 

To add or change a patient's Team Member

  1. Click +Select Team Member to reveal a drop-down of available Team Members within the patient's associated groups. Choose the Team Member you would like to add to the Care Team.
  2. Once you have two or more coaches affiliated with the patient, you may reveal the option to Set as a Primary Team Member. There may only be one Primary Team Member. You may have as many Support Team Members as available within the group.Screen_Shot_2017-10-31_at_4.28.12_PM.png
  3. At any time, you may remove the coach(es) affiliated with the patient:Screen_Shot_2017-10-31_at_4.27.41_PM.png
  4. Once you are satisfied with your choices, click Update Care Team.
