How do I see what my patients changed in their Health Action Plan?

When patients edit their own Health Action Plans you will automatically receive a Plan Change notification in your message stream:

You will also be able to reference the plan change in the Notes tab:

When patients make a plan change, they are prompted to complete a free text note describing that change so that you can build of their autonomy with continuous coaching:

If completed, you will be notified of the change in the patient message stream:

You may view and reference the note in the patient's Notes tab:
